Camp Health Form

• Health Form (Required)
• Adminstration of Medication (Required at camps held in Connecticut for Inhalers, Epi-Pen and all prescription and over-the-counter medication)

*Please bring the camp health form with you on the FIRST DAY of camp, do not mail ahead. In order to keep uniform documents on each camper we ask that you fill out OUR health form. If your doctor gives you, your athletes immunization records or a standard doctor summer camp form please staple it to our health form and write see attached for those portions of the form that apply.

**ONLY Camps Held in CT, NY & MA require a Dr. Signature on the camp health form.
